I work "most" days, but please be aware I prefer a day's notice to see me.  This gives me ample time to screen you and be prepared for our time together.  If you are someone I have previously seen, I will do my best to try and fit you into my schedule. My typical schedule is: 
Monday-Thursday: 11am-10pm
Friday: 12pm-8pm
Saturday: Advance Appointment only
Sunday: I don't work on Sundays

Please be aware my schedule does change often.  Appointments outside of my normal scheduled hours may be available with advance notice and subject to additional fees.

Please fill out the ENTIRE FORM.   If you only put your name and phone number I will reject your request.  I need at least one provider reference or a work reference.  YOU ARE NOT REQUIRED TO PROVIDE YOUR WORK INFO IF YOU HAVE AT LEAST ONE PROVIDER REFERENCE. This information is used ONLY for screening purposes and for my safety.  All information is destroyed after our appointment.  If you are unwilling to give me this information, I cannot see you.  NO EXCEPTIONS!!!!
